You will not be penalized for filing any complaint.
If you believe your rights with respect to health information about you have been violated by the DONIPHAN CO. HEALTH DEPT. / HOME HEALTH AGENCY, you may file a complaint with the DONIPHAN CO. HEALTH DEPT. / HOME HEALTH AGENCY or with the Secretary of the Department of Health and Human Services.
To file a complaint with the DONIPHAN CO. HEALTH DEPT. / HOME HEALTH AGENCY, contact the DONIPHAN CO. HEALTH DEPT. / HOME HEALTH AGENCY. All complaints must be submitted in writing.
